I, (full name below): HEREBY SIGN this Release of Liability/Promise Not to Sue to be permitted to participate in firearms training classes and instruction by PRESCOTT FIREARMS TRAINING.
I understand that this document is a release of liability AND is MY LEGAL PROMISE NOT TO SUE. I understand that by signing this "Release of Liability" I am giving up substantial legal rights to sue PRESCOTT FIREARMS TRAINING instructors, range safety officers, agents, sponsors and volunteers for negligence. I understand and have considered the dangers inherent in activities involving firearms. I understand and acknowledge that firearms are dangerous; and that any defensive training, whether utilizing firearms training, training weapons, hand-to-hand skills, knives or other normal and improvised weapons can be dangerous activities.
I acknowledge that I have the legal authority to be in legal possession of the firearms that I will be shooting (whether owned or borrowed), and am in compliance with all federal, state and local laws involving firearms.
I recognize that physical injury, serious physical injury, or in some cases even death may result in defensive training exercises. I recognize and acknowledge that precautions will be taken to ensure the safety and well-being of the participants in activities and events, but I nevertheless acknowledge that I am/my child or ward is giving up the right to sue for any negligent act(s) which may result in injury or death to me/my child or ward as a participant.
I release, discharge, and agree to hold harmless and indemnify Larry Hansen, dba Larry Hansen Firearms Instructor, and Larry Hansen dba PRESCOTT FIREARMS TRAINING, range safety officers, helpers, volunteers, sponsors, and all of them, from any and all liability claims, demands, causes of action, and possible causes of action whatsoever, arising out of or related to any loss, damage, injury or death, that may be sustained by me, my child or ward, which may accrue to heirs, survivors, next of kin, or personal representative while engaged in, on, in route to, from, or out of activities or events from any cause whatsoever related to such gun training.
I affirm that I am/my child or ward is in adequate physical health and condition to participate in this activity. I am/my child or ward is sufficiently physically trained and have/has the skills necessary to safely participate in this activity or event. I recognize and assume the inherent risks associated with this type of physical event and that my/my child's or ward's participation might result in physical injury or death.
I understand that PRESCOTT FIREARMS TRAINING instructor reserves the right to cease my firearms training if he feels I am being unsafe, or unwilling to obey safety or range rules, protocols, or for any
other reason they deem may be unsafe, in which case I agree to exit the range with no refund of instructor or range fees.
I HAVE READ THIS DOCUMENT AND UNDERSTAND THAT THIS is MY LEGAL PROMISE NOT TO SUE the above-described entities and individuals. I understand this is a legally binding release and indemnity for all claims and lawsuits. I understand that if I have any questions about the legal consequences of this Release of Liability or a reluctance to sign, I can choose not to participate in gun training activities with Larry Hansen, dba Larry Hansen Firearms Instructor and Larry Hansen dba PRESCOTT FIREARMS TRAINING, and will not be permitted to participate.
